Student Transcripts
To obtain an academic transcript of your school attendance in Napa Valley Unified School District, please follow these steps.
Step 1
Complete the Transcript Request Form Download
-OR-
Create a document with the following information:
- The name under which you attended school;
- The name of the school you last attended at NVUSD;
- Your birthdate;
- The year you graduated or left school;
- A daytime phone number;
- An address where you want the transcript sent;
- Your signature on the request;
- A check or money order for $2.00 per transcript payable to NVUSD.
Make sure you sign the request.
Step 2
Mail the completed form and payment to:
